The GMHBA Health Risk Assessment is an online survey that works simply by logging onto to www.gmhba.com.au and answering a series of multiple choice questions about your health.

In order to obtain the most accurate Health Risk Assessment, gather your personal medical statistics from your local GP at your next annual medical examination. Ensure you obtain the following information:

• Blood pressure
• Glucose level
• Cholesterol level

You can still complete the test and gain an accurate Health Risk Assessment even without the above information. Remember, if you don’t have Internet access at home just visit your local library.

Once you have completed all of the questions, the Health Risk Assessment will analyse your answers and calculate the results. Areas where your health may be at risk will be instantly highlighted in a summary report. Importantly, under GMHBA’s Privacy Policy you can be assured any information you disclose regarding your health will be kept confidential.

Research shows, most Australians do not have a thorough understanding of their health status. At GMHBA, we are committed to increasing your understanding and awareness and ultimately improving your health.

By completing the Health Risk Assessment, not only are you improving your health risk awareness, but you are providing us with valuable information on the health status of our membership.

The type of information we will obtain includes:

• How many Members smoke(ed) and how many cigarettes they smoke(ed) per day?

• Which quitting method was the most successful?

• How many Members have been diagnosed with or are at risk of Diabetes?

• How many Members are at risk of preventable illnesses?

• How often do Members eat fruit and vegetables?

• How often do Members drink alcohol?

• Do Members exercise regularly?

• What are the high-risk areas for our Members?

• And most importantly, how can GMHBA help?
